Though universal, we experience the loss of those close\nto us in singular ways; it is a suffering that only those who suffer can\ndescribe. <strong>Though grief has no time, it does have a place and a purpose: to\norganise our interior and exterior worlds, which have been overturned in the\nabsence of the one we love. <\/strong>The grieving process can prompt questions,\ninduce misgivings, cause a breakdown in day-to-day routines, trigger physical\nand mental health issues, and be a period of adapting to a new reality in the\nabsence of a loved one. <\/p>\n\n\n\n<p class=\"wp-block-paragraph\">Therefore,\nit is essential that care providers display empathy and demonstrate an ability\nto accompany those grieving a loss. To that end, Casa de Sa\u00fade da Idanha has\ndeveloped an intimacy programme for those who have lost someone during their\nhospitalisation, in an effort to facilitate the grieving process. The programme\nemerged organically from the palliative care unit, based on the objectives and\nphilosophy of that type of care. Subsequently, the programme spread to all\ninpatient units and care homes, because it is our belief that care does not die\nwith the patient but continues in the form of supporting relatives and\npreserving therapeutic (and humane) ties with the people the institution\nwelcomes.<\/p>\n\n\n\n<h3 class=\"wp-block-heading\">Multidisciplinary care plan<\/h3>\n\n\n\n<p class=\"wp-block-paragraph\">In this\ncontext, intervention is based on accompanying those in mourning as they adapt\nto the loss, a process that begins even before death occurs. In palliative\ncare, due to the severity of the prognoses, death is almost always expected. It\nis in this phase of severe illness that family members realise death will occur\nin a relatively short period of time. The loss of autonomy, the loss of the\nability to communicate verbally, physical changes to the person, and changes in\nsocial and family roles also symbolise losses in the run-up to a death and can\ncause anticipatory grief. At the same time, the need to make decisions and the\ntrials that family members encounter over the course of an illness can\nexacerbate suffering; this requires attention and monitoring in due time and can\naffect one\u2019s experience of grief. <\/p>\n\n\n\n<p class=\"wp-block-paragraph\">In\nthese moments, care professionals can ease the process by helping family\nmembers assimilate through systematised, multidisciplinary and fundamentally\nhuman and existential intervention. The team promoting the programme consists\nof a clinical psychologist and a nurse specialised in mental health; both have\nexpert training in the grieving process. They analyse each case and develop an\nindividual intervention plan, which is shared and updated with input from the entire\nmultidisciplinary team. In the anticipatory grieving phase, the opportunity\nexists to create a time and place for people to say goodbye, which often has a\npositive effect on the experience of grief. <\/p>\n\n\n\n<p class=\"wp-block-paragraph\"><strong>A\nspecific multidisciplinary care plan in the anticipatory grieving phase can\nhave a decisive impact<\/strong><strong> on people\u2019s lives by mediating forms of complex grief. <\/strong>In this way, we prioritise visits from the family members of\nhospitalised patients and encourage their participation in care (after\nconducting meticulous and rigorous assessments on the benefits for both patients\nand family members in the performance of these tasks). Issues that may\nexacerbate suffering are also addressed in each individual case. Psychotherapeutic\nfollow-up in this phase is extremely important in order to address any risk\nfactors that may cause future complications in the grieving process after the\npatient\u2019s death.&nbsp; &nbsp;<\/p>\n\n\n\n<p class=\"wp-block-paragraph\">At the\nsame time, patients also sense changes resulting from the disease and undergo\ntheir own process of preparatory grief as they confront their own finiteness,\noften reviewing their lives over and over and redefining their current\nexperience. &nbsp;<\/p>\n\n\n\n<h3 class=\"wp-block-heading\">The process of adapting to loss<\/h3>\n\n\n\n<p class=\"wp-block-paragraph\">After a\nloss, survivors experience bereavement in a unique way and it is important that\nthe care team is available and present during this process. When death occurs,\nthe team members inform the family and stay with them in order to help in any\nway possible. The family is informed of the team\u2019s intention to call some two\nmonths after the patient\u2019s passing. Meanwhile, our professionals send a\npersonalised letter, reflecting on the deceased and their life, expressing\nsympathy and condolences for the loss and including some guidelines to spot a\nlevel of suffering that may require further attention. The letter states that a\nprofessional will be in touch by phone shortly, and that the entire team is\navailable to help anyone in the grieving process. <\/p>\n\n\n\n<p class=\"wp-block-paragraph\">Telephone\ncontact is intended primarily to conduct a clinical interview to assess outward\nexpressions of grief that may indicate a more complex form of bereavement\ninvolving significant suffering that warrants clinical care. During these phone\nconversations, in which the bereaved interact freely with healthcare\nprofessionals, important clinical data is extracted about how the individual is\ncoping with their loss. This information determines the level of follow-up that\nthe professional will carry out, in accordance with the individual\u2019s wishes and\navailability. It is possible to change contacts when the situation justifies,\nand to arrange in-person consultations with a specific healthcare professional\nin order to clarify any issues that have persisted since the hospitalisation\nperiod, or to refer the person to a specialised bereavement counsellor and\/or\ncommunity resource specialising in these situations. Above all,<strong> it is\npossible to help the person based on their needs and experience of loss. <\/strong><\/p>\n\n\n\n<p class=\"wp-block-paragraph\">It is\nworth emphasising the extreme importance of training in this specific area.
